Accessing Housing Services

If you are homeless and need to access housing services, the first step is to contact Phoenix CDS’ Homeless Outreach Team by:

calling our office at 309-674-7310 or
visiting our office at 202 NE Madison Avenue, Peoria during regular business hours: Monday – Friday 8:30 am – 4:30 pm.
